Latest update on Global treatment of AIDS?

What has happened since 2004 on the situation between pharmaceutical companies and treatment of AIDS in poor countries? Are there any patents issues?

"The field of vaccine development today is shaped as much by philanthropy as by competitive market interests, global institutional arrangements of intellectual property rights, patents, scientific capacities (or the lack thereof), and the interests of investors and shareholders." Which leaves little interest in the rest of us. Don't ya think?
Mostly, it is still a grim story. The HIV treatments (combination therapies) that work are expensive and there is limited distribution of these treatments to poor countries. The pharmaceutical manufacturers have made some efforts to make the drugs available on a limited basis, but not a lot of effort. Their main concern is still profit.The countries are poor so they are not in a position to negotiate favorable rates for their people.HIV and AIDS continue to spread in poor countries.
